  • Reimbursement Form- If you have incurred an expense on behalf of Bellydancing UW, you may fill out this form for reimbursement and submit it to our treasurer. Submitting a request does not guarantee full reimbursement. Reimbursement does not apply to personal purchases; Bellydancing UW will only reimburse members for services we have requested and/or for which we have already agreed to reimburse club members (consult our organization's bylaws for further criteria).Coming Soon: Video and Costume Check Out Forms, BUW Bylaws
